I have read the above information carefully and have filled out this application honestly. I understand that omission of information and/or failure to answer all questions and sign the application can result in this application being declined. I give Sarah Papay permission to fully investigate the information provided as well as contact veterinarians and related persons. If the application passes this review, I agree to a home and yard visit on a mutually agreed date before an adoption decision is made.
In addition, I understand the adoption decision is dependent on many factors, and other applications received on this animal. I understand it is Sarah's decision to determine which home is most appropriate and that her decision is final, and therefore I will not argue with the decision. Unless otherwise indicated by her, I am free to apply and undergo the application process in the future. Please note once an dog is adopted, they may be returned for any reason but there will be no refunds for the adoption fee. I also understand this rescue is 100% non-profit and the animal's fee is soley for transport/shots/spay/neuter. I will bring the animal to my vet upon adoption for further needed shots and evaluation.
